Arrival Time

To allow time for check-in and registration, please complete your new patient paperwork online before arriving at the office, and plan to arrive at least 20 minutes before your scheduled appointment.

If you aren't able to complete the paperwork in advance, please arrive 30 minutes early so our staff has time to help you finish it. You're also welcome to bring a family member or friend to assist - and our staff is always glad to help as well.

If you're running late, please contact the office as soon as possible so we can do our best to accommodate you and our other scheduled patients. In some cases, late arrivals may be asked to reschedule so that every patient receives the full time they need with Dr. Cohen.

Please bring a valid photo ID and your insurance card (if you have one).

If you were treated by another physician for the same problem, it is very helpful to obtain the following documents prior to your visit:

Documentation of previous treating physicians / surgeons
All x-rays, CAT Scans, MRI's and/or lab tests
Documentation of previous therapists

If you have splints, please bring them with you.
